Client Service Consent - read carefully 

At Chrome Salon, we strive to provide exceptional services and use professional products to maintain the integrity of your hair. However, both chemical services (including but not limited to color, lightening, toning, perms, relaxers, and keratin treatments) and haircutting services carry inherent risks. Chemical treatments can alter the structure of the hair, and haircutting involves permanent change to length and shape that may not be reversible. These risks are heightened when hair is already compromised or previously chemically treated.

Hair Condition Acknowledgment 

I acknowledge that my stylist has evaluated the current condition of my hair and has informed me of any risks associated with performing a chemical service. I further acknowledge that any risks or damage resulting from a chemical service are inherent to the process, and I agree that neither my stylist nor Chrome Salon can be held responsible or liable for such outcomes.

Results Disclaimer

I understand that while my stylist will make every effort to achieve my desired results, the outcome may be limited by the current condition and history of my hair. Additional sessions may be necessary to achieve the desired look.

Risk of Damage

I release Chrome Salon and its stylists from responsibility for any damage to my hair including, but not limited to, dryness, breakage, scalp irritation, or undesired color results that may occur as a result of this service. I also acknowledge that haircutting services involve permanent changes to the length and shape of my hair, and I release Chrome Salon and its stylists from responsibility for any dissatisfaction or perceived damage related to haircut length, style, or outcome.

Allergies & Medical Disclosure

I confirm that I have informed my stylist of any allergies, scalp sensitivities, or medical conditions that may affect the outcome or safety of this service. I understand that Chrome Salon and its stylists cannot be held responsible for any reactions or adverse outcomes resulting from undisclosed or unknown allergies or sensitivities.

Agreement

By checking the Terms and Conditions box for online booking or paper copy at Chrome Salon, you acknowledge that you accept the risks associated with all salon services (including chemical treatments, haircutting, styling, and product use) and release Chrome Salon and its stylists from liability for any undesired results, damage to my hair, or loss/damage to my person or personal belongings.
